Student Incident Report

Sometimes, things happen at school that you might not like. It might be something that happens in class or in the yard, such as being left out of a game, hearing mean things or even being hurt.

When something like this happens, the best idea is always to ask the teacher on yard duty to help you out. You might also ask your class teacher or Miss Read to help you fix the situation. If you haven’t had a chance to talk to a teacher during the day, you can use the form on this page to let Miss Read know that something has happened that you need an adult to help you with.

You’ll need to leave your name and an email address (this could be your parents’ email, you should really talk to them about filling out this form so they can also help you).

Then you need to tell us a bit about what happened, by including the names of any students who were involved and what actually happened.

After this, you can write what you have already tried (maybe you spoke to the yard duty teacher) and the really important section is to let Miss Read know what you would like to happen next. Do you want her to come and speak to you? Do you want her to tell your teacher about what is happening? Maybe you need a quiet time to talk to another student without a lot of other people around. Sometimes the teachers will have to make a decision about what needs to happen next, but it is important for you to let us know how we can help you.

After this, you just click submit and the message goes through to Miss Read. It is important that you know that she may not see your message until the next school day. This is why it is important that you also talk to your parents about what is bothering you if you have the chance. Your report will be followed up on the next school day.
